Best Tirzepatide Providers in Texas

Most Texans pay cash for tirzepatide: Texas Medicaid, the state employee plan and TRS plans for school employees don't cover it for weight loss. So the best tirzepatide providers in Texas pass three checks you can run yourself: a prescriber with a Texas license, a pharmacy licensed by the Texas State Board of Pharmacy, and one clear monthly price. Online programs that serve Texas charge about $129 to $399 a month for compounded tirzepatide. MaxLife costs $120 a month on a Starter Kit plan, then $195 a month, with no membership fee. Brand Zepbound® costs $299 to $449 a month from LillyDirect. Compounded tirzepatide is not FDA-approved.

Who wrote this guide

MaxLife, a telehealth company based in Irvine, California, publishes this guide and is one of the options in it. We serve Texans by mail. We list every price and rule with its source and the date we checked it (October 2026), and we show our own fine print too.

Does insurance cover tirzepatide in Texas in 2026?

Usually not for weight loss. Texas Medicaid, the big public employee plans and many job-based plans exclude weight-loss drugs, so most Texans who use tirzepatide pay cash. Many have no plan at all: in 2025, 16.5% of Texans, about 5.2 million people, had no health insurance, the highest rate of any state and about twice the U.S. rate of 8.4% (U.S. Census Bureau).

There are real exceptions, though: sleep apnea, heart risk, the Texas A&M System plan, TRICARE for military families and a new $50 Medicare program. Find your plan in the table.

If your plan says no

Ask your plan for the reason in writing. If a state-regulated plan denies a drug as not medically necessary, the Texas Department of Insurance's independent review costs you nothing. The decision comes within 20 days, or within 3 days for a drug you are already taking. An independent review can't overturn a plan exclusion, though, and it doesn't cover self-funded employer plans, ERS, TRS, UT or Medicaid plans, which have their own appeals.

Don't wait for a new law. In 2025 the Legislature let two coverage bills die: HB 2412, for health plans, never left committee, and HB 2677, for Medicaid, stalled on the House calendar.

Paying cash in Texas? Start at $120 a month

MaxLife compounded tirzepatide: $120 a month on a Starter Kit plan, then $195 a month. No membership fee. A licensed provider decides.

Check if you qualify

Can you get tirzepatide online in Texas without a video visit?

Yes, when the provider reviews real clinical information. Some websites and AI answers say Texas always requires a live video call. The law says otherwise. Texas Occupations Code §111.005 lets a provider licensed in Texas start your care with "asynchronous store and forward technology," meaning they review information you send instead of meeting you live, as long as they use "the patient's relevant clinical records, such as the relevant medical or dental history, laboratory and pathology results, and prescriptive histories" or clinically relevant photos.

The same law adds three duties. The provider must meet the same standard of care as an office visit, give you guidance on follow-up care and, if you agree and have a primary care doctor, send that doctor a report within 72 hours. On the pharmacy side, the Texas State Board of Pharmacy tells pharmacists to look into a prescription they suspect was written only from the answers to a questionnaire (22 TAC §291.29(c)).

In practice, a careful Texas program asks for your full health history and medication list, may ask for labs, explains how follow-up works, and calls you when something needs a closer look.

Texas telemedicine law

What Texas law requires of an online visit

Occupations Code §111.005 and the boards' rules, in plain words.

  • AllowedNo video needed when records are reviewed

A store-and-forward visit is valid when the provider uses your relevant clinical records or images (Occupations Code §111.005(a)(3)(B)).

What it means for you: Expect detailed health questions and a medication list, not a two-question quiz.

  • LicenseYour prescriber needs a Texas license

Anyone who prescribes to you while you are in Texas must hold a Texas license. Texas joined the Interstate Medical Licensure Compact on September 1, 2021, which speeds up licenses for doctors from other states.

What it means for you: Look your doctor up on the Texas Medical Board site.

  • NPs and PAsNurse practitioners and PAs work with a Texas doctor

They prescribe under a prescriptive authority agreement with a Texas physician (Occupations Code Chapter 157).

What it means for you: Ask which Texas physician the program's nurse practitioners work with.

  • Follow-upFollow-up and your regular doctor

Store-and-forward providers must explain follow-up care and, with your consent, send your primary care doctor a report within 72 hours. Texas telemedicine providers must also show the Texas Medical Board's complaint notice on their website or app.

What it means for you: Say yes to the report if you want your regular doctor in the loop.

This is general information, not legal advice.

Which pharmacies can make compounded tirzepatide for Texans?

Compounded tirzepatide is an injection, so the pharmacy needs a sterile compounding license from the Texas State Board of Pharmacy. A Texas pharmacy needs a Class A-S license. A pharmacy in another state needs a Class E-S license: Texas inspects it before issuing the license and in every renewal period, and since 2016 its pharmacist in charge must hold a Texas pharmacist license. A plain community or nonresident license (Class A or Class E) doesn't cover compounded injections.

These pharmacies cluster in the big metros. In October 2026, the Board's license file listed 380 active in-state sterile compounding pharmacies in 53 of Texas's 254 counties, 73 of them in Harris County, plus 153 out-of-state pharmacies licensed to ship sterile compounds here, most often from Florida, Arizona and New Jersey. In the other 201 counties there is no local one, so the medicine comes by mail.

5-minute check

How to check a Texas pharmacy and prescriber

  1. Find the pharmacy's name

Ask the program in writing which pharmacy fills your prescription, or read the label: Texas requires the pharmacy's name, address and phone number on it.

  2. Search the Board's site

Open the Texas State Board of Pharmacy's Pharmacy Search and type part of the name or the license number.

  3. Check the class

Community Sterile Compounding (Class A-S) for a Texas pharmacy, or Non Resident Sterile Compounding (Class E-S) for one in another state.

  4. Check status and address

The status should be Active, the expiration date in the future, and the address the same as on your label.

  5. Check the prescriber

Look up your doctor on the Texas Medical Board site, and nurse practitioners or PAs on their own boards' sites.

Filling a brand Zepbound® prescription in Texas

Brand Zepbound® is the FDA-approved tirzepatide for weight management, and Lilly's cash prices are the same in Texas as everywhere else. You need a prescription from any licensed prescriber; the medicine is billed separately from the visit.

  • LillyDirect: vials or KwikPens® cost $299 at the starting dose, $399 at the next step and $449 at higher doses per 28 days, as long as each refill comes within 45 days. Later refills cost $499 or $699. Delivery is free, or pick it up free at a Walmart pharmacy.
  • Other pharmacies, including H-E-B, CVS and Walgreens: Lilly's KwikPen® self-pay card brings the price to the same $299, $399 or $449 through December 31, 2026, for up to 11 fills a year. H-E-B, based in San Antonio, is not one of LillyDirect's pharmacies, so call your H-E-B pharmacy first to ask whether it stocks the KwikPen® and takes the card.
  • TrumpRx: lists Zepbound® vials at the same price and sends the order to LillyDirect.
  • Medicare: if you qualify for the GLP-1 Bridge, the Zepbound® KwikPen® costs $50 a month from July 1, 2026 to December 31, 2027.

Low-cost prescribers that serve Texas include Walgreens Weight Management at $49 a visit. Add the prescriber's fees to the medicine price when you compare it with a compounded program.

Buying tirzepatide in Mexico: what Texans should know

It rarely saves money, and the risks are real. In Mexico tirzepatide is sold as Mounjaro®, by prescription. A month of it cost about 4,400 to 9,700 pesos in Mexican pharmacies in February 2026, roughly $255 to $565 depending on the dose (Posta). That is close to or above Lilly's U.S. cash prices of $299 to $449.

  • Fakes: Mexico's drug regulator, COFEPRIS, warned in February 2026 and again in October about illegal tirzepatide sold online. One product labeled tirzepatide held semaglutide and bacteria.
  • Bringing it back: U.S. Customs and Border Protection says it is illegal in most cases for U.S. citizens to bring home drugs bought abroad, and the FDA says personal importation is illegal "in most circumstances."
  • Heat: tirzepatide has to stay cold, and a summer drive back from a border pharmacy can ruin it.
  • Travel: the U.S. State Department rates Tamaulipas, home to Nuevo Laredo, Reynosa and Matamoros, "Do Not Travel."

One Texas rule surprises people. A Texas pharmacist may fill an original paper prescription from a doctor licensed in Mexico for a drug that isn't a controlled substance, such as tirzepatide (Health and Safety Code §483.001). The U.S. price still applies, so it doesn't make the medicine cheaper.

Keeping tirzepatide cold through Texas heat, hurricanes and freezes

Heat is the everyday risk. Lilly's label lets you keep Zepbound® out of the fridge for up to 21 days, but only at or below 86°F (30°C). In summer 2026, every Texas airport weather station we checked passed 86°F on at least 104 of the 122 days from June through September. McAllen was above it on all 122 days and reached 100°F on 97 of them.

What to do about it

  • Plan deliveries for when you're home, and bring the box inside right away. Texas pharmacies must pack mailed medicine to hold the right temperature, must not ship a drug that could be damaged in transit and must replace a delivery that arrives compromised (22 TAC §291.12). If a box arrives warm, call the pharmacy before you use the medicine.
  • Never let it freeze. Keep ice and gel packs from touching the pen or vial, because freezing ruins it. That matters in a Texas freeze too: Winter Storm Uri left millions of Texans without power in February 2021, and the state health department linked some of its deaths to frozen medical devices and medicine.
  • In a power outage, keep the fridge closed and have a cooler with frozen gel packs ready. Hurricane Beryl left about 2.26 million CenterPoint customers around Houston without power in July 2024, and more than 88,000 were still out a week later (Texas Tribune).
  • After a disaster declaration, a Texas pharmacist may give you up to a 30-day emergency supply in the counties it covers. The Board of Pharmacy allowed this in May 2024, July 2024 and July 2026.
  • Compounded tirzepatide follows its own pharmacy label, which is often stricter than the brand label.

How tirzepatide prices have changed

Brand prices have come down and compounded prices have settled. Lilly's list price for Zepbound® is about $1,087 a month, but cash buyers rarely pay it: LillyDirect's starting-dose price fell from $399 in August 2024 to $349 in February 2025 and $299 in December 2025. Compounded tirzepatide from online programs that serve Texas costs about $129 to $399 a month in October 2026.

What MaxLife offers Texans

MaxLife is a telehealth company based in Irvine, California, and we serve all 50 states, Texas included. Our program offers compounded tirzepatide from $120 a month on a Starter Kit plan, with no membership fee and the same price at higher doses. Your provider review, ongoing support, medicine if prescribed, supplies and free overnight shipping in an insulated container are included. In Texas we work with a licensed partner pharmacy authorized to serve Texas.

A licensed provider reviews your online intake and health history, usually within 24 hours. Texas law doesn't require a video visit when the provider reviews your clinical records, so you won't need one unless your provider asks for a call or more information.

More than a quarter of Texans, about 8.2 million people, speak Spanish at home. MaxLife has Spanish-speaking staff who can guide you through the program and support you during treatment.

MaxLife is a patient management platform, not a pharmacy or a drug maker. Independent licensed providers decide whether a prescription is right for you, and completing the intake does not guarantee one.

Yes, when a Texas-licensed provider prescribes it for your individual needs and a pharmacy with a Texas sterile compounding license makes it. Pharmacies may not make mass copies; the Fifth Circuit upheld the FDA's position on August 27, 2026. Compounded tirzepatide is not FDA-approved.
Yes. Texas Occupations Code §111.005 allows a store-and-forward visit when the provider reviews your relevant clinical records, such as your medical history, labs and prescription history. Some programs still use video, so ask how yours reviews your history.
Yes. Anyone who prescribes to you while you are in Texas needs a Texas license, and you can look doctors up on the Texas Medical Board's website. Nurse practitioners and physician assistants also work under an agreement with a Texas physician.
No. Texas Medicaid and CHIP exclude weight-loss drugs. Since May 2, 2025, Medicaid has covered Zepbound® with prior authorization for adults with obesity and moderate to severe obstructive sleep apnea.
Only if your employer added it. BCBS of Texas says weight-loss drugs are not a standard pharmacy benefit when Prime Therapeutics manages the benefit. Call the number on your card and ask whether your plan covers weight-loss medicines.
Not for weight loss. HealthSelect excludes drugs for weight reduction; it covers GLP-1 drugs for diabetes and has covered Wegovy® for heart risk since September 1, 2024.
Not for weight loss. TRS says its plans have always excluded weight-loss drugs. Diabetes medicines are covered.
It depends on the system. UT SELECT excludes GLP-1 drugs except for diabetes. A&M Care's documents list Wegovy® and Zepbound® with prior authorization through Express Scripts, so ask Express Scripts to confirm.
Yes, for TRICARE Prime and Select members, with prior authorization and a prescription from a network provider. TRICARE For Life does not cover weight-loss drugs.

Yes, with a prescription, if your store stocks it. H-E-B is not one of LillyDirect's pharmacies, but Lilly's KwikPen® self-pay card brings the price to $299 to $449 at other pharmacies through December 31, 2026. Call your H-E-B pharmacy first.
In most cases, no. CBP says U.S. citizens generally can't import drugs bought abroad, and the FDA says personal importation is illegal in most circumstances. Mexico's drug regulator has also warned about fake tirzepatide.
Search for the pharmacy on the Texas State Board of Pharmacy's website. It needs an active Community Sterile Compounding (Class A-S) license if it is in Texas, or a Non Resident Sterile Compounding (Class E-S) license if it is in another state.
Yes, if it holds a Texas Class E-S license. Texas inspects these pharmacies and requires a Texas-licensed pharmacist in charge.
Keep it in the fridge; brand Zepbound® can stay out for up to 21 days only at or below 86°F. Bring deliveries inside quickly, keep the fridge closed in an outage and use a cooler with gel packs that don't touch the medicine. Ask the pharmacy to replace a box that arrives warm.
