This Consumer Health Data Privacy Policy supplements the MaxLife Privacy Notice, available here. This Consumer Health Data Privacy Policy applies to personal data defined as “consumer health data” subject to the Washington My Health My Data Act and the Nevada Health Privacy Act and similar U.S. laws governing consumer health data.
“Consumer health data” is personal information that is linked or reasonably linkable to a consumer and that identifies the consumer's past, present, or future physical or mental health status.
To provide the products, services or features offered by MaxLife Technologies Inc. and its affiliated medical groups (“Company”, “we”, “us”, “our” and such products, services or features, the “Company Offerings”), we collect and process information about you, including information that could be considered Consumer Health Data. Depending on how you interact with the Company and the Company Offerings, we may collect Consumer Health Data about you directly from you (which may include your interactions with our website and the Company Offerings), from other people (including other users of the website and Company Offerings), your device(s), and partners, vendors and third parties, as described further in our Privacy Notice.
We collect and use Consumer Health Data as reasonably necessary to provide you with the Company Offerings you have requested. This may include:
