Semaglutide Before and After: Weight Loss Results and What to Expect
Medically reviewed by Dr. Kavya Patel, MD. Pricing is accurate as of November 2026 and subject to change.
Key Takeaways
- In STEP 1, adults without diabetes taking Wegovy 2.4 mg lost an average of 14.9% of body weight over 68 weeks (about 35 pounds from a 232-pound start), compared with 2.4% on placebo.
- In STEP 4, people who reached the 2.4 mg dose had lost an average of 10.6% by week 20; the Wegovy label reports no separate average for the first month.
- Weight loss leveled off around weeks 60 to 65 and held at 15.2% at 104 weeks in STEP 5; in the STEP 1 extension, people who stopped regained about two-thirds of their loss within a year.
- Wegovy HD (7.2 mg, approved March 2026) produced 18.8% versus 15.5% on 2.4 mg over 72 weeks, and the Wegovy pill (25 mg daily) produced 13.6% over 64 weeks.
- Ozempic is approved for type 2 diabetes, not weight loss; in the diabetes trials in its label, average losses were about 3.5 to 6.4 kg (8 to 14 pounds).
- These are brand trial results: they have not been shown for compounded semaglutide, which is not FDA-approved, and individual results vary.

Semaglutide weight loss builds over months, not weeks. In the STEP 1 trial, adults without diabetes taking Wegovy (brand semaglutide) 2.4 mg once a week lost an average of 14.9% of their body weight over 68 weeks, compared with 2.4% on placebo. For someone who starts at 232 pounds, close to the trial average, that is about 35 pounds. Early change is smaller: in STEP 4, people who reached the 2.4 mg dose had lost an average of 10.6% by week 20. As of November 2026, the higher 7.2 mg dose and the daily Wegovy pill have results of their own, covered below. These results come from trials of the brand product, have not been shown for compounded semaglutide, and individual results vary.
This guide builds a realistic timeline from the trials' own numbers, week by week where the studies allow it, with no patient photos or testimonials. Wegovy and Ozempic are made by Novo Nordisk. Compounded semaglutide is prepared by a licensed pharmacy for one patient's prescription and is not FDA-approved, which means the FDA hasn't reviewed it for safety, effectiveness or quality.
Quick Answer: How Much Weight Can You Lose on Semaglutide?
In the main trials of Wegovy, average weight loss ranged from about 10% to 19% of body weight, depending on the dose, the form, how long people took it and whether they had type 2 diabetes. Everyone in these trials also got diet and activity counseling, as the Wegovy prescribing information describes.
| Trial (Wegovy) | Who and what dose | Average weight change |
|---|---|---|
| STEP 1, 68 weeks | 1,961 adults without diabetes, 2.4 mg weekly | -14.9% (placebo -2.4%) |
| STEP 2, 68 weeks | Adults with type 2 diabetes, 2.4 mg weekly | -9.6% (placebo -3.4%) |
| STEP 3, 68 weeks | 2.4 mg weekly plus intensive behavioral therapy | -16.0% (placebo -5.7%) |
| STEP 5, 104 weeks | Adults without diabetes, 2.4 mg weekly | -15.2% (placebo -2.6%) |
| STEP UP, 72 weeks | Adults with obesity, 7.2 mg or 2.4 mg weekly | -18.8% or -15.5% (placebo -3.9%) |
| OASIS 4, 64 weeks | Adults without diabetes, 25 mg pill daily | -13.6% (placebo -2.4%) |
In STEP 1, 83.5% of people taking Wegovy lost at least 5% of their weight, 47.9% lost at least 15% and 30.2% lost at least 20%, compared with 31.1%, 4.8% and 1.7% on placebo. On the 7.2 mg dose in STEP UP, 45.5% lost at least 20%.
These numbers belong to brand Wegovy and have not been shown for compounded semaglutide. If cost is your next question, our semaglutide cost guide compares brand and compounded prices. Through MaxLife, compounded semaglutide costs $89 a month, flat, if an independent licensed provider prescribes it; it is not FDA-approved.
Semaglutide Before and After: A Realistic Weight Loss Timeline
Weight loss on semaglutide is gradual because the dose starts low and rises for 16 weeks before it reaches 2.4 mg, so most of the change comes after the first few months. When you look at semaglutide before and after photos online, remember that they show selected people, not averages.
| Time on treatment | What the Wegovy trials measured |
|---|---|
| Weeks 1 to 4 (0.25 mg) | No separate average; this is the starting dose |
| Weeks 5 to 16 | Dose steps up through 0.5 mg, 1 mg and 1.7 mg |
| Week 17 | First 2.4 mg dose on the fastest schedule |
| Week 20 | Average loss of 10.6% among people who reached 2.4 mg (STEP 4) |
| Week 24 | 21% had lost at least 15% (SURMOUNT-5, Wegovy group) |
| Weeks 60 to 65 | Weight loss levels off (STEP 5, SELECT) |
| Week 68 | Average loss of 14.9% (STEP 1) |
| Week 104 | Average loss of 15.2% (STEP 5) |
| Week 208 | Average loss of 10.2% in adults with heart disease (SELECT) |
The sections below walk through each stage. Use them as a guide, not a scorecard: in every trial, some people lost much more than the average and some much less.
Weeks 1 to 4: The Starting Dose
In the first month, you take 0.25 mg once a week, and the goal is getting used to the medicine rather than reaching a number on the scale. The Wegovy label says to follow the step-up schedule "to reduce the risk of gastrointestinal adverse reactions."
The label says semaglutide "decreases calorie intake," likely by affecting appetite, so you may feel full sooner from the start.
How much weight can you lose on semaglutide in the first month? The trials don't report a separate average for the first 4 weeks, and claims of "5 to 10 pounds in month 1" don't come from them. Some people see a small drop on the scale in the first weeks; others see little change until the dose rises.
Stomach side effects are common at this stage. In the Wegovy weight-loss trials, nausea affected 44% of people on Wegovy and 16% on placebo, and the label says these reactions "were most frequently reported during dosage escalation." Smaller meals and enough fluids can help. Tell your provider if side effects make it hard to eat or drink.
Months 2 to 4: When Do Results Start to Show?
For many people, measurable weight loss shows up in months 2 to 4, as the weekly dose rises to 0.5 mg, 1 mg and 1.7 mg. STEP 4 gives the clearest early number: 902 adults started Wegovy, and the 803 who reached 2.4 mg by week 20 had lost an average of 10.6% of their weight by then. For someone who starts at 236 pounds, the STEP 4 average, that is about 25 pounds.
That figure comes from people who made it through the step-ups: about 11% of those who started stopped during the first 20 weeks, most often because of side effects. If a step is hard to tolerate, the label lets your prescriber wait another 4 weeks before the next one.
If you use compounded semaglutide through MaxLife, an independent licensed provider sets each step and follows up with you along the way, and you can ask for a video visit if a step is hard. The pharmacy label on your vial tells you how much to draw.
How Long Does It Take to Lose 20 Pounds on Semaglutide?
The trials don't report a time to lose 20 pounds, so think in percentages. For someone who starts at 232 pounds, 20 pounds is about 8.6% of body weight. Against the STEP 4 average of 10.6% at week 20, that works out to roughly 4 to 5 months on the full schedule. If you weigh less, 20 pounds is a larger share of your weight and may take longer. Your own pace can differ a lot from the average.
Months 5 to 12: Steady Progress, Then a Plateau
Months 5 to 12 are when most people are on a maintenance dose of 1.7 mg or 2.4 mg and the losses add up. In a SURMOUNT-5 analysis, 21% of people taking Wegovy had lost at least 15% of their weight by week 24. In STEP 1, the average reached 14.9% at week 68.
Then the curve flattens. In STEP 5, average weight loss "plateaued after approximately week 60." In the trials, leveling off after about a year was the usual pattern.
If you have tolerated 2.4 mg for at least 4 weeks and your provider decides you need more weight reduction, the Wegovy label now allows up to 7.2 mg a week, sold as Wegovy HD since the FDA approved that dose in March 2026. In STEP UP, the 72-week trial behind it, adults lost 18.8% on 7.2 mg and 15.5% on 2.4 mg, compared with 3.9% on placebo. Changes in skin sensation, such as tingling or burning (dysesthesia), were more common on 7.2 mg: 22%, compared with 6% on 2.4 mg.
Your habits matter most at this stage. Eating enough protein and staying active help you keep muscle while you lose fat; see our guides to protein on a GLP-1 and exercise on a GLP-1. Look at your trend over a whole month, not week to week.
Year 2 and Beyond: Keeping the Weight Off
In the trials, staying on treatment is what kept the weight off. In STEP 5, adults were still 15.2% lighter on average at 104 weeks, compared with 2.6% on placebo. In the SELECT trial of 17,604 adults with heart disease and overweight or obesity but no diabetes, weight loss continued to week 65 and held at an average of 10.2% after 4 years, compared with 1.5% on placebo.
Stopping changes the picture. In STEP 4, people switched to placebo after week 20 regained 6.9% of their weight over the next 48 weeks, while those who kept taking Wegovy lost another 7.9%. In the STEP 1 extension, people who stopped Wegovy and the lifestyle program at week 68 regained about two-thirds of the weight they had lost within a year.
Talk to your provider before you stop, and plan together for the dose and routines you can keep.
What Affects Your Semaglutide Weight Loss Results?
Dose, diabetes, lifestyle support and staying on treatment all changed the average results in the Wegovy trials. Starting weight, sleep, stress and other medicines may also play a part, so individual results vary.
Dose
Higher maintenance doses brought larger average losses: 15.5% on 2.4 mg and 18.8% on 7.2 mg in STEP UP. They also brought more side effects for some people. The label lists 1.7 mg or 2.4 mg as maintenance doses and tells prescribers to weigh your response and how well you tolerate it, so the best dose is the one you can stay on.
Type 2 Diabetes
People with type 2 diabetes tend to lose less on average. In STEP 2, adults with type 2 diabetes lost 9.6% on 2.4 mg over 68 weeks, compared with 3.4% on placebo. In the 7.2 mg trial for adults with type 2 diabetes, losses were 13.2% on 7.2 mg and 10.4% on 2.4 mg, compared with 3.8% on placebo.
Diet, Activity and Muscle
More lifestyle support added to the result. In STEP 3, which paired Wegovy with a low-calorie diet for the first 8 weeks and 30 counseling visits, adults lost 16.0%, compared with 5.7% on placebo with the same program. The label says semaglutide "lowers body weight with greater fat mass loss than lean mass loss." Aim for protein at each meal, enough fiber and water, and some strength training a few times a week.
Pill or Injection
The Wegovy pill, approved by the FDA on December 22, 2025, produced an average loss of 13.6% over 64 weeks at 25 mg a day in OASIS 4, compared with 2.4% on placebo. It needs the same routine every morning: on an empty stomach with up to 4 ounces of plain water, then wait at least 30 minutes before food, drinks or other pills. The pill and the injection were tested in separate trials, not head to head.
Ozempic Before and After: Is Ozempic Approved for Weight Loss?
No. Ozempic is FDA-approved for adults with type 2 diabetes, to improve blood sugar and lower the risk of heart and kidney problems, not for weight loss, according to the Ozempic label. Wegovy is the semaglutide brand approved for weight management, at higher weekly doses (2.4 mg or 7.2 mg, compared with Ozempic's 2 mg maximum).
So how much weight can you lose on Ozempic? In the diabetes trials described in its label, adults lost an average of about 3.5 kg to 6.4 kg (8 to 14 pounds) over 30 to 56 weeks, depending on the dose; the largest was 6.4 kg on 2 mg over 40 weeks. That is less than in Wegovy's weight-loss trials, which used higher doses; Ozempic's trials enrolled only people with type 2 diabetes, who tend to lose less on average. Photos labeled "Ozempic before and after" show selected people, not these averages. Our post on semaglutide vs Ozempic explains how the brands differ.
What about "Ozempic face"? It is a popular term for a thinner-looking face after weight loss. It is not a term used in the Ozempic or Wegovy labels, and the trials didn't measure it. The Wegovy label does list hair loss, which it links to weight reduction: 3.3% of people on 2.4 mg and 5.8% on 7.2 mg reported it, compared with about 1% on placebo. If changes in your face, skin or hair worry you, raise them with your provider.
Before and After Photos: Why the Scale Is Not the Whole Story
The scale is only one way to measure progress, and it doesn't always show changes right away. Waist size fell along with weight in the trials: by an average of 13.5 cm (about 5.3 inches) on 2.4 mg in STEP 1, compared with 4.1 cm (1.6 inches) on placebo, by 17.5 cm (6.9 inches) on 7.2 mg in STEP UP and by 12.2 cm (4.8 inches) on the Wegovy pill in OASIS 4.
This article doesn't show before and after photos: a photo is one person's chosen result, while trial averages describe everyone who took part. Your own photos are different, because they compare you with your own starting point. Take them every 2 to 4 weeks in the same light and clothes. Measure your waist and hips once a month, and notice how belts, rings and jeans fit. Several ways of tracking give a truer picture than the scale alone, and they help you stay motivated in weeks when the number barely moves.
Semaglutide vs Tirzepatide: How Results Compare
In a head-to-head trial, tirzepatide led to more weight loss than semaglutide. In SURMOUNT-5, an open-label trial funded by Eli Lilly, adults with obesity and without diabetes lost an average of 20.2% on Zepbound (tirzepatide 10 or 15 mg) and 13.7% on Wegovy (1.7 or 2.4 mg) over 72 weeks. By week 24, 44% of people on tirzepatide and 21% on semaglutide had lost at least 15%.
Wegovy has strengths of its own: a heart-risk indication, approval from age 12 for the injection, a daily pill and the 7.2 mg dose, which SURMOUNT-5 didn't test. See tirzepatide before and after for that timeline and tirzepatide vs Wegovy for the full comparison.
Are These Results Realistic for You?
The trial averages are a guide to what is possible, not a prediction for you. People in STEP 1 started at an average of 105.3 kg (about 232 pounds) with an average BMI of 37.9, 74% were women, none had diabetes, and all of them got diet and activity counseling. Even in that trial, 16.5% of people taking Wegovy lost less than 5% of their weight.
Wegovy injection is FDA-approved, together with a reduced-calorie diet and more physical activity, for adults with obesity, adults with overweight and at least one weight-related condition, and children 12 and older with obesity; the pill is for adults. Semaglutide is not for people with a personal or family history of medullary thyroid carcinoma or MEN 2, and the label says to stop it at least 2 months before a planned pregnancy.
Only a licensed provider can decide whether semaglutide is right for you. Our BMI calculator for weight loss gives a trial-based estimate, not a prediction.
How to Start Semaglutide With a Licensed Provider
The right route depends on your coverage and what you need. If your insurance covers Wegovy, or you want the pill, the 7.2 mg dose or the heart-risk indication, brand Wegovy is the product with these trial results, so ask your provider about it. Without coverage, NovoCare Pharmacy sells Wegovy pens self-pay for $199 for each of a new patient's first two fills, then $349 a month, and the Wegovy pill for $149 to $299 a month by dose.
If you're paying cash and want a lower monthly cost, MaxLife offers compounded semaglutide through independent licensed providers. You complete a short online intake, about 2 minutes, and a provider reviews your health history and goals, usually within 24 hours; you can ask for a video visit. If semaglutide is right for you, the provider sets your starting dose and follows up as you go, and higher doses are available when a provider prescribes them, at the same price. A licensed partner pharmacy prepares and dispenses it, with free expedited shipping.
It costs $89 a month, flat, billed every 3 months ($267). There's no membership fee and no insurance needed, and the price includes the provider's review and follow-up, the medicine if prescribed and supplies. MaxLife is a telehealth patient management platform, not a pharmacy or drug maker. Compounded medications are not FDA-approved, the trial results in this article have not been shown for compounded semaglutide, and results are not guaranteed.
Conclusion
Semaglutide before and after results are a slow climb, not a quick fix. The first month is an adjustment phase at 0.25 mg with no trial average to aim for. By week 20, people who reached 2.4 mg in STEP 4 had lost 10.6% on average, and the average reached 14.9% at 68 weeks in STEP 1 before leveling off. The 7.2 mg dose raised the average to 18.8%, and the daily pill produced 13.6%.
Results depend on the dose you reach, type 2 diabetes, your habits and staying on treatment, and Ozempic is not approved for weight loss. Track photos and measurements as well as your weight, and work with a licensed provider. These are brand trial results, not shown for compounded semaglutide, and individual results vary.
